Troy, Mich. —January 11, 2018—Practice Transformation Institute (PTI), a non-profit organization providing continuing medical education and customized learning programs that improve health outcomes and the individual care delivery experience, is pleased to announce it has received continuing education accreditation in social work for its Care Coordination and Care Management training program. The accreditation was approved by the Michigan Social Work Continuing Education Collaborative for 12.5 contact hours and is valid for one year, through January 18, 2019.
PTI’s Care Coordination and Care Management Training is designed to educate health care professionals on the specifics of primary care management. The purpose of the course is to provide interprofessional care team members with a knowledge base and skill set to enable care coordination and care management within a practice. The social work accreditation follows PTI’s accreditation for its Care Coordination and Care Management training in September 2017 from the Michigan Nurses Association, an approver of continuing nursing education by the Michigan Board of Nursing.
“PTI continues to gain acceptance from a variety of healthcare disciplines for a training curriculum that complements primary care, acknowledging that care coordination and care management training produce measurable benefits for patients who are seen along the healthcare spectrum from both clinical and non-clinical providers,” Ewa Matuszewski, PTI principal and founder, said.
