Troy, Mich. —July 30, 2014—Practice Transformation Institute (PTI), providing the health care community with continuing medical education and customized learning programs that improve patient health outcomes and the individual care delivery experience, is offering the Personal Action Toward Health (PATH) workshop series to community members at no charge beginning Aug. 20 in Madison Heights.

PATH is Michigan’s version of the acclaimed Stanford Chronic Disease Management Program. The six-week workshop offers tips on day-to-day living to help participants improve their health, feel better and be better able to face the challenges of living with an ongoing health condition. The workshop focuses on the following key areas:

• Dealing with not feeling well
• Talking with health care providers, friends and family members
• Relaxing and overcoming stress
• Increasing energy levels
• Accomplishing everyday tasks more easily
• Staying independent
