Rochester, Mich. – October 15, 2020 – MedNetOne Health Solutions (MedNetOne)—a health care management organization offering infrastructure, clinical and technology services to nearly 1,000 private practice physicians and other independent care providers including behavioral health specialists—is now offering onsite primary care to individuals at The Salvation Army Harbor Light Macomb facility in Clinton Township. Harbor Light offers an entire continuum of substance abuse treatment services, including in-patient medication assisted detoxification, 14- to 90-day residential treatment, outpatient treatment, case management, peer recovery services, legal aid, and pastoral care.

The opportunity with Harbor Light Macomb stems from MedNetOne’s primary care partnership with community service agency Judson Center, a designated Certified Community Behavioral Health Clinic (CCBHC). Judson Center’s many services include behavioral health and integrated primary care through Judson Center Health.  Judson Center Health opened in 2019 at Judson Center’s Warren facility through a partnership with MedNetOne, with a primary care physician and nurse practitioner employed by MedNetOne.

In September, Judson Center entered a partnership with The Salvation Army Eastern Michigan Division to extend the primary healthcare services offered at Judson Center Health to an onsite facility at Harbor Light Macomb. The health goals of the partnership between Salvation Army Eastern Michigan Division and Judson Center, with assistance from MedNetOne, are to reduce hospital and out-of-home placement readmissions for individuals who have been treated for mental health or substance use disorder, deliver outpatient mental health services to those who have not received treatment in the prior six months, reduce critical events such as hospitalizations, suicide, and overdoses, and track health effectiveness data and information set (HEDIS) measures on a quarterly basis for clients served.

MedNetOne is involved with a variety of broad-based primary care initiatives, including the Patient-Centered Medical Home, the Patient-Centered Medical Home Neighborhood and the High Intensity Care Management (HICM) model for the frail, elderly and homebound population.  The organization is actively involved in healthcare and community education efforts to raise awareness of the role that Social Determinants of Health (#SDOH) play in overall health and wellness.
