Rochester, Mich. – January 25, 2019 – MedNetOne Health Solutions (MedNetOne), a health care management organization offering infrastructure, clinical and technology services to more than 900 private practice physicians and other independent care providers, including behavioral health specialists, has been selected by Judson Center to provide clinical oversight to its new integrated primary care clinic, Judson Center Family Health Clinic, opening February 1, 2019 in Judson Center’s Warren location.
Judson Center is a multi-county human service agency providing autism programs, behavioral health services, child and family services including foster care and adoption in tandem with its affiliate, Child Safe Michigan, and employment services for adults with developmental disabilities. The organization received nearly $700,000 in grant funding in support of its planned integrated primary care clinic, Judson Center Family Health Clinic, and turned to MedNetOne to provide clinical oversight, technology, infrastructure and health programming.
The new clinic will provide primary care health services integrated with mental health care to existing Judson Center consumers, primarily children from low-income backgrounds, as well as surrounding communities, and will serve all ages from infants to seniors. A 2018 survey of Judson Center’s behavioral health patients indicated that many were not presently seeing a primary care physician. Psychological barriers faced by people with mental health challenges often make it difficult for them to navigate the health care system, keep appointments, and prioritize their health care.
“MedNetOne is privileged to be collaborating with Judson Center on this forward thinking approach to whole person care,” Ewa Matuszewski, CEO and co-founder of MedNetOne, said. “Judson Center has been part of MedNetOne’s Patient-Centered Medical Home Neighborhood (PCMH-N) for several years, and this opportunity to join forces for the purposes of advancing primary care where it’s most needed is an example of the benefits to patients when complementary healthcare organizations seek joint solutions to complex community health challenges.”
All patients receiving primary care services at Judson Center Family Health Clinic will also get a mental health risk assessment. Additionally, MedNetOne will offer Diabetes Prevention Programs (DPP), Diabetes Self-Management Education (DSME), and multidisciplinary, team-based wellness services, including MedNetOne’s RTEAM, a program for pediatric and adolescent patients that provides nutrition and exercise counseling and a behavioral health specialist to help create action plans and SMART goals. The clinic is comprised of both medical and behavioral health and referrals across medical and behavioral health services will be available.
Lenora Hardy-Foster, President and CEO of Judson Center, commented on the partnership between her organization and MedNetOne.
“Judson Center has spent 95 years finding solutions to help families succeed. The opening of Judson Center Family Health Clinic represents the next evolution of services we can provide to accomplish our mission and also serve the broader community that may not be currently acquainted with Judson Center,” Hardy-Foster said. “With MedNetOne’s clinical expertise and decades-long knowledge of primary care practices, this collaboration will have an immediate impact on those with unmet primary care needs.”
MedNetOne is involved with a variety of broad-based primary care initiatives, including the Patient-Centered Medical Home, the Patient-Centered Medical Home Neighborhood and the High Intensity Care Management (HICM) model for the frail, elderly and homebound population. The organization is actively involved in healthcare and community education efforts to raise awareness of the role that Social Determinants of Health (#SDOH) play in overall health and wellness.
In April 2018, MedNetOne partnered with the Oakland University School of Health Sciences to jointly present a first of its kind #SDOH symposium in Michigan titled “Better Upstream Health for Better Downstream Care. The symposium will return on May 1, 2019, with the theme #SDOH: It Takes a Team.
