Rochester, Mich. –August 14, 2018 –MedNetOne Health Solutions (MedNetOne), a health care management organization offering infrastructure, clinical and technology services to more than 800 private practice physicians and other independent care providers including behavioral health specialists, has been selected by Phoenix-based Solera Health to implement diabetes prevention programs for Blue Cross and Blue Shield of Michigan (BCBSM) and Blue Care Network Medicare Advantage plan participants.

MedNetOne is certified to offer the national diabetes prevention program (NDPP) by the Centers for Disease Control (CDC). NDPP is a lifestyle and diabetes intervention program aimed at preventing Type 2 diabetes. Working with Solera, the sole national NDPP vendor selected by BCBSM and other Blue Cross Blue Shield Association (BCBSA) affiliates, MedNetOne’s lifestyle coaches will engage with patients throughout the state to coach them on diabetes prevention strategies and lifestyle approaches that have measurable results in the prevention of Type 2 diabetes.

“MedNetOne is pleased to be working with Solera, BCBSM and Blue Care Network as enhancements to Medicare Advantage benefits make this important diabetes prevention program available to seniors seeking to avoid the life threatening and costly effects of Type 2 diabetes,” Ewa Matuszewski, CEO and co-founder of MedNetOne, said.

In addition to diabetes prevention programs and physician organization services, MedNetOne is involved with broad-based primary care initiatives including the Patient-Centered Medical Home, the Patient-Centered Medical Home Neighborhood and the High Intensity Care Management (HICM) model for the frail, elderly and homebound population.  The organization is actively involved in healthcare and community education efforts to raise awareness of the role that Social Determinants of Health (#SDOH) play in overall health and wellness. In April 2018, MedNetOne partnered with the Oakland University School of Health Sciences to jointly present a first of its kind #SDOH symposium in Michigan titled “Better Upstream Health for Better Downstream Care.
