MedNetOne’s focus on care management services, multi-disciplinary care teams and pandemic support elevates smaller physician practices with coordinated quality care; new to the list is integrated primary care practice Judson Center Health in Warren

Rochester, Mich. – October 15, 2020 – MedNetOne Health Solutions (MedNetOne)—a health care management organization offering infrastructure, clinical and technology services to nearly 1,000 private practice physicians and other independent care providers including behavioral health specialists—announced that 42 physician practices have been selected as a Patient-Centered Medical Home (PCMH) designated practice by Blue Cross Blue Shield of Michigan (BCBSM).  The announcement was made by MedNetOne CEO Ewa Matuszewski.

The PCMH model is founded on the central patient/physician relationship, with the primary care physician’s office serving as the patient’s “medical home” and the primary care physician’s team coordinating all of a patient’s medical care to improve patient health and outcomes.  Matuszewski notes that MedNetOne member physicians have received the PCMH designation because of a deeply held commitment to the principles of patient-centric care.

“The overwhelming majority of MedNetOne physician members are small, entrepreneurial practices that rely on the infrastructure and care management support that MedNetOne membership provides,” Matuszewski said. “Especially during the pandemic, as these dedicated physicians focus even more intensely on the physical and emotional well-being of their patients, we are privileged to work in partnership with them to help attain and continue their PCMH designation – with the broader goal of providing optimal, coordinated patient care.”

MedNetOne’s focus on care management includes services offered in the physician’s office by a patient-centered, multi-disciplinary team comprised of registered nurses, pharmacists, dieticians and behavioral health specialists. Working with the physician, they offer coordinate care and services to better manage chronic conditions such as diabetes and hypertension.

New to the PCMH designation list this year is Judson Center Health, an integrated primary care clinic which opened in 2019 through a joint venture with MedNetOne and community service agency Judson Center. Located in Judson Center’s Warren facility, it is led by a primary care physician and nurse practitioner from MedNetOne.  The entire list of MedNetOne member practices that have been PCMH designated can be found here.

The BCBSM announcement of PCMH designated practices was made on September 15. BCBSM’s PCMH program is the largest of its kind in the country, and its PCMH-designated practices consistently outperform non-designated practices in clinical quality and utilization measures. Learn more here.
