MedNetOne Health Solutions among 10 physician organizations collaborating on pharmacy and primary care program with Blue Cross Blue Shield of Michigan and University of Michigan Health System

Rochester, Mich. –Nov. 23, 2015 – MedNetOne Health Solutions (MNOHS), a health care management organization offering infrastructure, clinical and technology services to independent physician practices and other independent care providers, is one of 10 physician organizations across the state participating in the Michigan Pharmacists Transforming Care and Quality (MPTCQ) program. Less than half of adults take their medications as prescribed; MPTCQ is a new initiative to help patients manage and comply with their medication plans.

MPTCQ is a collaboration between Blue Cross Blue Shield of Michigan, the University of Michigan Health System, physician organizations and pharmacists across the state. With MPTCQ, clinical pharmacists work with Patient-Centered Medical Home (PCMH) practices within MNOHS and other selected physician organizations to become part of the medical home’s patient care team. Blue Cross has the largest Patient-Centered Medical Home program of its kind in the country and in July 2015, MNOHS announced that 56 of its member practices had received PMCH designation from Blue Cross.

With the MCPTQ program, pharmacists review patients’ medication lists, collaborate with physicians to make necessary medication adjustments and work with patients to help them understand how to safely and properly use the medications. Initially, pharmacists are focusing on patients with diabetes, high blood pressure and high cholesterol.

“This initiative aims to broaden the relationships between physicians and pharmacists, creating additional care coordination for the patient,” David Share, MD, MPH, senior vice president, Value Partnerships at Blue Cross said in a statement. “We’ll be developing effective care models. It extends the team caring for patients, and extends the reach of our Value Partnerships umbrella to include additional care providers such as pharmacists.”

After the initiative’s first year, all 46 physician organizations that participate with the Blue Cross PCMH program are expected to be phased in over a three- to four-year period.
