ROCHESTER, Michigan – July 1, 2013- MedNetOne Health Solutions, a Michigan-based health care management organization serving the infrastructure and clinical support needs of primary and specialty care private practice physicians and behavioral health specialists, is pleased to announce it is the recipient of a grant from the Michigan Public Health Institute (MPHI).

The grant is intended to build relationships with health care providers and health systems to increase ongoing referrals to National Diabetes Prevention Programs (NDPP) in the community. The grant also comes at a time when the Michigan Diabetes Prevention and Control Program (DPCP), with support from the National Association of Chronic Disease Directors (NACDD), is working to increase referrals to NDPP sites in Michigan.

MedNetOne Health Solutions’ Diabetes Self-Management Education (DSME) program is accredited by the American Association of Diabetes Educators and meets the American Diabetes Association national standards for Diabetes Self-Management Education. As a NDPP provider, MedNetOne Health Solutions will focus on health care provider practices in Oakland County, including those in its own physician network, to initiate referral processes to local NDPP sites. The organization will also collaborate with other diabetes prevention programs in the region to maximize resources and reduce overlap of services. The funding covers services provided from July 1 through Nov. 15.
