Rochester, Mich. – Dec. 16, 2015 – MedNetOne Health Solutions (MNOHS), a health care management organization offering infrastructure, clinical and technology services to independent physician practices and other independent care providers, announces that Warren, Mich.-based member practice Lifetime Family Care, PLLC was among 30 physician practices across Michigan recently receiving recognition for their achievements in patient care.
The practice, which was recognized in the category of best overall adult and family medicine, was selected from over 300 primary care practices statewide participating in the Michigan Primary Care Transformation Project (MiPCT). This category measured best performance in 2014 and assessed whether patients received appropriate screenings as well as the avoidance of unnecessary emergency department visits and hospital admissions. Jane Castillo, M.D. accepted the award on behalf of the practice.
The MiPCT is a five-year statewide effort to improve health in the state, make care more affordable and strengthen the patient-care team relationship. The program provides support to make patient care more coordinated and proactive through the use of Care Managers (specially-trained nurses and other care professionals). MedNetOne Health Solutions has been a member of MiPCT since its inception in 2012. Jean Malouin, M.D., Medical Director of the MiPCT project, complimented the practice winners.
